At Your Service Home Care proudly partners with South Carolina’s Community Long Term Care (CLTC) Medicaid program to provide eligible individuals with compassionate, reliable care in the comfort of their own homes.

What is CLTC?

Community Long Term Care (CLTC) is a South Carolina Medicaid program that helps seniors and individuals with disabilities access the services they need to stay safely at home, rather than moving to a facility.

Services covered may include:

  • Personal care and hygiene assistance

  • Light housekeeping and meal preparation

  • Medication reminders and health monitoring

  • Respite care for family caregivers

  • Ongoing support to maintain independence

We work directly with CLTC case managers to ensure services are tailored to each client’s care plan and needs.

Who Qualifies for CLTC?

To qualify, individuals must meet certain financial and medical eligibility requirements set by South Carolina Medicaid. The CLTC program is available to:

  • Seniors and adults with disabilities

  • Individuals who meet nursing home level of care

  • Those who choose to receive care at home instead of an institution

Eligibility is determined by a CLTC case manager after an assessment.
